Consent Forms
Field Trip Permission Slip
A blank permission slip for you to print out and return to school with your child.
Medical Release Form
This form allows Step of Faith staff to give medical aid to your child before you are able to be contacted; this is only used in situations where the issue is to serious to wait.
Student Pick-up Authorization
A list of people who are authorized to pick up your child from school.
Athletic Health Exam Form
Any student wishing to participate in our athletics programs must complete and turn in this form before they will be allowed to participate in practices.
